Our Severe Disability Assessment service is used to evaluate one’s ability to perform Activities of Daily Living (ADLs) independently. This assessment helps to determine your eligibility for financial assistance and plan long-term care needs in Singapore.

What is a Severe Disability Assessment (SDA)?

A Severe Disability Assessment evaluates an individual’s capability to independently perform Activities of Daily Living (ADLs). These essential tasks include:

 

To qualify for financial assistance, an individual must be certified as requiring full assistance with at least three of the six Activities of Daily Living (ADLs).

When will you or your family member need a Severe Disability Assessment?

You will need a Severe Disability Assessment if you are:

  1. Applying for long-term care financial assistance from the Agency for Integrated Care (AIC), which includes:
    • CareShield Life
    • ElderShield
    • ElderFund
    • Interim Disability Assistance Programme for the Elderly (IDAPE)
    • MediSave Care
    • Foreign Domestic Worker Levy Concession for Persons with Disabilities (FDWLC(PWD))
    • MediSave Withdrawal for LTC
    • Pioneer Generation Disability Scheme (PioneerDAS)
  2. Applying for supplementary schemes of CareShield Life and ElderShield through private insurers like Great Eastern, Income, or SingLife.

What is considered a Severe Disability?

An individual is considered severely disabled if they cannot independently perform at least three of the six Activities of Daily Living (ADLs):

  • Feeding
  • Dressing
  • Toileting
  • Washing
  • Walking or moving around
  • Transferring from bed to chair and vice versa
How long will a Severe Disability Assessment take?

A typical Severe Disability Assessment takes approximately 45 minutes or longer.

How is Severe Disability Assessment conducted?

During the assessment, doctors will review an individual’s medical history and observe their performance of specific tasks to evaluate their ability to carry out these activities.

Can all doctors perform Severe Disability Assessment?

Severe Disability Assessment cannot be performed by all doctors but only by doctors who are accredited by Ministry of Health to perform the assessment.

What are the fees for Severe Disability Assessment? Are there subsidy for the Severe Disability Assessment fees?

For information regarding the fees associated with Severe Disability Assessments and any available subsidies, please reach out to our clinics directly.

If you are assessed to be severely disabled, the full assessment fee will be reimbursed to you by government/ insurance company.

What happens after assessment is done?

Our clinic will submit the assessment report to Agency for integrated Care (AIC) or to your private insurer. AIC and your private insurer will inform you in writing of the outcome.
